{"id":188,"date":"2020-06-29T11:18:49","date_gmt":"2020-06-29T15:18:49","guid":{"rendered":"https:\/\/www.adelphi.edu\/health\/health-services-and-resources\/sexual-health-2\/"},"modified":"2021-08-09T09:16:00","modified_gmt":"2021-08-09T13:16:00","slug":"medical-clearance","status":"publish","type":"page","link":"https:\/\/www.adelphi.edu\/health\/health-services-and-resources\/medical-clearance\/","title":{"rendered":"Medical Clearance"},"content":{"rendered":"<h2><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-192 alignright\" src=\"https:\/\/www.adelphi.edu\/health\/wp-content\/uploads\/sites\/30\/2020\/06\/Medical-Clearence-Vaccines-Immunizations-300x150-1.jpg\" alt=\"\" width=\"300\" height=\"150\" \/>All Adelphi Students<\/h2>\n<p>As a new student, you must meet New York State immunization requirements and the<span>\u00a0<\/span><a href=\"https:\/\/www.adelphi.edu\/health\/\">Health Services Center<\/a><span>\u00a0<\/span>standards:<\/p>\n<ul>\n<li>A completed<span>\u00a0<\/span><a href=\"https:\/\/www.adelphi.edu\/health\/forms-and-requirements\/\">Health History and Assessment<\/a><span>\u00a0<\/span>form.<\/li>\n<li>Proof of<span>\u00a0<\/span><a href=\"https:\/\/www.adelphi.edu\/health\/forms-and-requirements\/measles-mumps-and-rubella-immunization-requirements\/\">immunization against measles, mumps and rubella<\/a><span>\u00a0<\/span>on the Immunization Requirements form.<\/li>\n<li>A completed<span>\u00a0<\/span><a href=\"https:\/\/www.adelphi.edu\/health\/forms-and-requirements\/meningococcal-disease-immunization-requirements\/\">Meningococcal Meningitis Vaccination<\/a><span>\u00a0<\/span>response form.<\/li>\n<\/ul>\n<p>These forms are sent out with the accepted student\u2019s packet and can be downloaded from the<span>\u00a0<\/span><a href=\"https:\/\/health.adelphi.edu\/forms\/\">Health Services Center<\/a><span>\u00a0<\/span>website or picked up from the health Services Center in Waldo hall. The forms must be completed and signed by your healthcare provider and returned prior to registration.<\/p>\n<address><strong>Please note:<\/strong><span>\u00a0<\/span>\u00a0Adelphi needs proof of required immunization.
